Providing Specialized CPO Support

The Community Programs Office is dedicated to supporting students and colleagues with their special needs, whenever possible. These are a comprehensive collection of forms and resources to support the UCLA community in engaging with the Community Programs Office. Each form is designed to facilitate access to CPO’s services, resources, and expertise.

CPO Departmental Funding & Resources Request Form

Please submit this form to smagpusao@cpo.ucla.edu. It should be used for requests involving in-kind contributions of supplies, equipment loans, or funding support.

CPO Outreach Request Form

This form helps students and colleagues request CPO’s participation in tabling at events, promoting events through CPO’s social media, or displaying marketing materials in the department.

CPO Design and Print Shop Requests

The technology request form is intended to support students, staff, and faculty with special requests from CPO Design Services and the CPO Print Shop.

CPO Speaker Request Form

This form is for requesting CPO staff members to speak at an event.
